Find answers to frequently asked questions about our services and accessing support. If your question isn’t answered here, please contact our team.

We provide in-person disability support and mental health services in Brisbane, Logan, Toowoomba, Gold Coast, Scenic Rim, Ipswich and surrounding areas, and via telehealth services Australia-wide.

We endeavour to respond to emails as soon as possible but for new enquiries, please allow up to 1-2 business days to get back to you.

To become a Footsteps client, you can either:

  1. Ask your support coordinator or medical professional to refer you*; or
  2. Contact us directly to start the process.

From there, we will liaise with you to determine service would suit you best, formulate a service agreement and organise for one of our therapists or practitioners to assist you with filling out the service agreement and intake forms.

* We have provided details on the process to follow under the Question, “I am a Support Coordinator/Medical Professional, how do I refer a Client?”

Referring a client to us is easy. Simply download and complete our:

  • General Client Referral Form; or
  • Respite Client Referral Form

Once received, we will decide whether we are the right organisation to take on your client. If we feel we can not help them, we will refer you to an organisation that can.

In most cases, we are more than happy to help, and will formulate a service agreement for you and your client to look over.

Absolutely! Whether you have NDIS funding, health insurance, or are paying out of pocket, anyone can use our services.

As well as being a registered NDIS provider, we are registered to provide services to BUPA, Medibank Private, AHM and ARHG clients, as well as WorkCover and NIISQ recipients.

We also regularly work with government departments including Child Safety and Disability Services and Victim Assist, depending on the needs and circumstances of our clients.

If you think you might be eligible for the NDIS, you can download an Access Request Form (ARF), and a Supporting Evidence Form (SEF) and work with your GP, allied health, and specialist medical professionals to prepare and provide the information required.

We are also here to help you navigate, understand, and access the scheme. Ways we can support you include:

  • Assessing if you are likely to be eligible for NDIS funding
  • Assisting you to understand the NDIS
  • Providing you with tools to help you prepare for the NDIS
  • Identifying goals that the NDIS will consider to be funded
  • Assisting you to understand your plan

We regularly help clients to apply for the NDIS. Our friendly, knowledgeable team can assess if you are likely to be eligible for the NDIS, and complete the necessary assessments to assist you in gaining NDIS funding.

Other ways we can support you include:

  • Providing you with tools to help you prepare for the NDIS
  • Identifying goals that the NDIS will consider to be funded
  • Assisting you to understand your plan

Disability Support & Mental Health Services for Individuals & Families

Endorsing your registered NDIS provider on PACE

If you work with a registered NDIS provider like Footsteps, you will need to contact the NDIS and tell them which provider you work with.  This is so that they can connect with your plan in the PACE system, and is known as endorsing your provider.

You can endorse your provider:

If your NDIS Plan is on the new PACE system and Footsteps is already your registered NDIS provider, you can simply call the NDIA on 1800 800 110 and ask them to endorse Footsteps Community Services (provider registration number 4050065204) as your provider.

A guardian or nominee can also contact the NDIA to endorse Footsteps on your behalf.
