FAQ

What is the best way to contact Footsteps Community Services?

You can contact us either at admin@footstepstherapy.org or on 0467 374 123. If you choose to email us, please allow us 1-2 business days to get back to you.

What is the process to becoming a client?

To become a Footsteps Community Services’ client, you can either ask your support coordinator or medical professional to refer you (using the process listed below) or you can contact us to get the ball rolling. From there, we will figure out which service would suite you best, formulate a service agreement using a person centred approach, and organise one of our practitioners or therapist to help you fill out the service agreement, along with a few other intake forms.

Can I use your services if I am not an NDIS participant?

Absolutely! Whether you have NDIS funding, health insurance, or are paying out of pocket, anyone can use our services. And if you would like, we can assess if you would be eligible for the NDIS, and complete the necessary assessments to gain NDIS funding.

How do I apply for the NDIS?

If you think you might be eligible for the NDIS, you can download an Access Request Form (ARF), and a Supporting Evidence Form (SEF), work with your GP, allied health, and specialist medical professionals to prepare and provide the information required. To find out more, and to download the forms required, you can find them at https://www.ndis.gov.au/applying-access-ndis

We are here to help you navigate, understand, and access the scheme. We can help you in the following ways:

  • Assist you to understand the NDIS.
  • Provide you with tools to help you prepare for the NDIS.
  • Identify goals that the NDIS will consider to be funded.
  • Assist you to understand your plan.
I am a support coordinator/ medical professional, how do I refer a client?

Referring a client to us is very easy. Simply download either our General Client Referral Form or our Respite Client Referral Form (found on our front page), fill it out, and send it to us at admin@footstepstherapy.org

After this, we will decide to either take on your client or if we feel we can not help them, refer you to an organisation that can. In most cases, we are more than happy to help, and will formulate a service agreement for you and your client to look over.
